- Documents to change the company name with the social insurance agency include:
- Declaration of participating units and adjustment of social insurance and health insurance information (Form TK3-TS).
- Copy of Business Registration Certificate with new name.

Instructions for preparing TK3-TS declaration to change company name with the social insurance agency:
(01). Unit name: write full business name.
(02). Unit code: write the unit code issued by the social insurance agency
(09). Establishment decision/Business registration license with new company name:
(09.1). Number: write the number of the establishment decision/business registration license.
(09.2). Place of issue: write the agency that issued the Establishment Decision/Business Registration License to the unit.
(10). Other payment methods (only applicable to enterprises, cooperatives, individual business households, and cooperative groups operating in the fields of agriculture, forestry, fishery, and salt production that pay wages by product or by piece): if you choose the payment method every 03 months, mark x in box (10.1); If you choose the payment method every 6 months, mark x in box (10.2).
Note: When changing information about units participating in social insurance, health insurance, and unemployment insurance, you only need to write down the unit tax code and criteria (01), (02), (09), (10).
